My client in Mesa is looking to hire part time Case Managers to start Nov. 23rd!

Job Description:

**This is a contract role that will run through the end of the year. They anticipate taking top performers on perm depending on business needs. NOT REMOTE OR WFH**
- Completes all actions required for active case management including, but not limited to, benefit eligibility and disbursement, creating and maintaining client files, follow-up actions on the client's status, recommending program opportunities
- Check phone messages, call clients, and do assessments by phone and make sure clients are income eligible and inform them of all documents required
- Developing and maintaining collaborative relationships with consumers, their family members and significant support systems
- Scheduling, facilitating, and/or participating in Family/Individual, Case Management as appropriate. Updating and completing staffing notes

Schedule: Monday through Friday 5pm-9pm/ Optional Saturdays from 8am-5pm

Pay: $15-16/hr

Qualifications: Open to Experience
