-Refers participants and their families/friends to appropriate community agencies.
-Serves as a liaison and advocate with such organizations to assure participants receive full range of services to which they are entitled.
-Participates in the interdisciplinary team planning conferences to develop an integrated, individualized plan of care.
Brings participant input into the care planning process.
-Interprets fiscal assessment of eligibility and fees, if any, to participant and family/friends as needed. provide technical assistance to participants on managing finances.
-Coordinates needs during transition periods with participants, family and PEPS staff. Assist in placement of participants in long term care facilities and maintain contact with participants to assure continuity of care.
-Responsibilities to Multidisciplinary Team: Maintains communication with the Social Services Manager concerning caseload issues and develop processes to provide social work coverage during absences. Maintains close working relationships with center supervisors, physician and clinic staff, rehab staff, and personal care aides.
-Administrative/Record-Keeping: Maintains current written social work records and meet all administrative requirements, e.g., statistical records and service reports required monthly, quarterly, and annually. medical records. staff meetings. Maintains all participant information and records in a professional and confidential manner, per HIPAA regulations.
Top Skills Details:
1) Masters in Social Work (MSW)- will POSSIBLY consider Bachelor's Degree but would need special approval and would not be guaranteed
2) 1+ years of experience working with adult and/or geriatric patient population (social work experience specifically)
3) Washington State Agency Affiliated Counselor or LIACSW or LAASW or LICSW or LASW (preferred, might accept without)
Additional Skills & Qualifications:
-Looking for someone that has had experience working with adult and/or geriatric patient populations doing discharge planning as a social worker specifically
-Current CPR certification.
