Perform member outreach and coach members into healthy habits and gap closures regarding coverage and provider visits. Assist with membership attribution, provider and patient engagement, and solving for SDOH concerns. Provide resources to members to improve health outcomes.

Work Site
This is a fully remote position.
Work Environment
M-F 8-5. This job is remote and can sit in TN or AR. Tools to perform job duties, including laptops, phone, and training guides, will be provided. Performance Expectations: 32 calls/day/160/wk; Reach rate of 20% of members called (6 members/day) to educate on preventative measures and assist members with scheduling Annual Wellness Visits.
