Pharmacist

Description:

This is an inpatient pharmacist role with a large hospital. The pharmacist will be doing rounds with patients, doing pharmacokinetic calculations, will be communicating with the on-call pharmacist regarding approvals for restricted-use medications, and provide clear communication of necessary information sufficiently enough for a decision of the requested drug. Will follow up notes in PCIS or verbally communicate with fellow staff members. Where applicable performs order entry for hand-written physician orders. Quality and safety-these are evaluated using such parameters as necessary interventions with the prescribing physician; monitoring of medication allergy and drug interactions by patient profile review; ability to retrieve necessary data for safe medication practice using PCIS, and properly recomposing PCIS entry errors whenever it has occurred; correct interpretation and application of departmental P&P; successfully completes the aseptic technique training session once yearly.
