URGENTLY HIRING 10 Discharge Coordinators in Northridge, CA

ON SITE Mon-Fri 8am-5pm

With a well known Medical Group in the Valley

Description:

We are going to need 10 discharge coordinators to help support the nursing staff for our inpatient case management team. This project/temp assignment is strictly for us to help the LA CM team and Case Management in general from an administrative standpoint with duties that are not currently performed by our team and they cannot currently absorb.

Skills:

Data entry, Customer service, Medical terminology, Schedule appointments, Insurance verification

Additional Skills & Qualifications:

• Benefit Check for certain requested items, by calling HP and obtaining PA form when applicable.

• PLOF discussion with member/family

• Review Continued Stay Authorizations (# of days approved/when last day is and notify CM when additional days are needed)

• Communicating DME delivery time and HH SOC to hospital CM/DC Planner

• Census vs Assignment Reconciliation

• EMR – Retrieve Medical Records & Upload

• Follow-up with CM to ensure Orders/TO for ancillary services are sent to coordinators for timely processing
