Job Description

The Registered Nurse (RN) is responsible for delivering and coordinating compassionate care for individuals with terminal illnesses and their families, whether in private homes, skilled nursing facilities, or residential care settings. As the case manager, the RN leads the interdisciplinary care team, determining the frequency of visits and ensuring the care plan is effectively implemented. The role focuses on providing comfort, education, and clinical interventions to enhance quality of life.


Responsibilities

  • Conduct holistic assessments of patients and families, considering physical, emotional, spiritual, social, and cultural factors from initial referral through bereavement.
  • Deliver direct nursing care within the RN scope of practice, adhering to professional and regulatory standards.
  • Collaborate with the interdisciplinary team to develop, evaluate, and revise individualized care plans based on changing needs.
  • Coordinate and oversee services provided by internal team members and external partners to ensure alignment with the care plan.
  • Participate in interdisciplinary group (IDG) meetings to support collaborative care planning.
  • Maintain timely and accurate documentation of assessments, interventions, outcomes, and communications in the patient’s medical record.
  • Perform clinical procedures as trained and outlined in clinical guidelines.
  • Participate in the on-call rotation to provide after-hours nursing support as needed.
  • Provide continuous care during crises to support patients and families.
  • Facilitate access to community resources and coordinate services across disciplines.
  • Use downtime productively to support future care needs and team readiness.
  • Maintain effective communication with all personnel involved in patient care, including informal and formal meetings.
  • Represent the hospice philosophy in community outreach and educational efforts.
  • Develop and supervise aide care plans in accordance with regulatory requirements.
  • Engage in team meetings, committees, and quality improvement initiatives.

Skills & Qualifications

  • Preferred: At least one year of professional nursing experience; prior home health or hospice experience is a strong plus.

Work Environment

This full-time RN role provides hospice care across a rural service area that includes Adams, Decatur, Ringgold, and Union counties. Care is delivered in patients’ homes and skilled nursing or residential care facilities. The position is based out of the Mt. Ayr location, where the team is known for its compassionate, collaborative approach to patient care. RNs in this role manage a caseload of approximately 12 to 15 patients, with caseloads adjusted based on patient acuity and individualized care needs. The schedule is Monday through Friday, 8:00 AM – 5:00 PM, with flexibility to accommodate family and personal needs. Occasional on-call coverage may be required during staff absences or high-acuity patient needs. Regular travel between patient locations is expected, with mileage reimbursement provided biweekly.
