Nurse Care Manager

Description:

These RNs would be responsible for a host of patient care needs in clinic Monday- Friday with one expected late night per week. This RN will receive notification in the EHR from internal provider referrals that a patient needs to be enrolled in the self monitoring blood pressure program. The RN will coordinate that enrollment by adding to trackers, telephonic outreach, face to face teaching and monthly check in. This RN will focus on patients with uncontrolled hypertension as opposed to all chronic disease management. There may be some travel between the location required.

Additional Skills & Qualifications:

The best RN for this role has recent experience in a clinic environment like a urgent care or even an ER and has experience with referrals.

Do NOT need case management experience.

Strong candidates have experience with infectious disease and chronic illnesses like hypertension and diabetes.

Must have current RN License

EMR experience is a plus
