Registered Nurse Case Manager

Description:

Full-time Registered Nurse working with diabetic patients and managing their care plan. They would follow up with their patients and manage their numbers. Schedule their follow up appointments with specialist, notify their primary care provider of updates. Follow up with the patient and make sure they are taking their medicine correctly. This is a great opportunity for an experienced RN with excellent assessment skills with Ambulatory and/or Emergency Room experience. In addition to the duties listed, all opportunities require a passion to serve the community. This individual would be working predominantly in their call center and helping triage patients to the appropriate places.

Additional Skills & Qualifications:

• Graduate of an accredited School of Nursing or Bachelor Degree in Nursing (BSN) with a valid Missouri Nursing License.
• Must have three years of experience in ambulatory care or experience in Women's Health, pediatric or public health setting desirable.
