RN Case Manager *URGENT HIRE*

Description: Full-time Registered Nurse working with diabetic patients and managing their care plan.

• Follow up with patients and manage numbers.
• Schedule follow up appointments with specialist, notify primary care provider of updates.
• Follow up with the patient and make sure they are taking their medicine correctly.

This is a great opportunity for an experienced RN with excellent assessment skills with Ambulatory and/or Emergency Room experience. In addition to the duties listed, all opportunities require a passion to serve the community. This individual would be working predominantly in the call center and helping triage patients to the appropriate places.

Additional Skills & Qualifications:

Graduate of an accredited School of Nursing or Bachelor Degree in Nursing (BSN) with a valid Missouri Nursing License.

Must have three years of experience in ambulatory care or experience in Women's Health, pediatric or public health setting desirable.
