Case Manager (Registered Nurse)

Great opportunity for a RN looking to serve the diabetic community and assist patients through triaging and managing care plans.

Description:

Full-time Registered Nurse working with diabetic patients and managing their care plan.

• Follow up with their patients and manage their numbers.
• Schedule their follow up appointments with specialist and notify primary care provider of updates.
• Follow up with the patient and make sure they are taking their medicine correctly.

In addition to the duties listed, all opportunities require a passion to serve the community. This will primarily be a triage position with little to no patient care.

Additional Skills & Qualifications:

Graduate of an accredited School of Nursing or Bachelor Degree in Nursing (BSN) with a valid Missouri Nursing License.

Must have three years of experience in ambulatory care or experience in Women's Health, pediatric or public health setting desirable.
