Registered Nurse Case Manager- Bilingual

Hiring for a Registered Nurse with Case Management/ Disease Management Experience to work remotely!

Requirements:

• Must have a valid COMPACT Registered Nurse license
• Must have at least 3+ years of nursing experience in Case Management and Disease Management.
• Strong team-oriented mindset with a passion for patient care,
• Strong computer skills 

Responsibilities -including but not limited to:

• Telephonic case management for patients - which includes patient education and managing continuous care.
• Accomplishes patient care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress.
• Responsible for complying with policies and procedures under nursing laws and regulations
