Job Description: 

• Plans, directs, coordinates, delegates and evaluates the activities of all nursing personnel within assigned area.
• Reviews care plans for appropriate goals, problems, interventions, and revises as needed, based on identified nursing needs.
• Ensures the continuity of care by informing supervisors and staff of unit status and related matters through written reports and verbal communication.
• Conducts daily conferences with unit staff to review specific patient care, and to communicate and interpret recent policy and procedure changes.
• Maintains records, files, and reports as directed.

Qualification :

• Must Holds active LPN License from State of NY
• 6+ Months of experience working as LPN is Mandatory
• Experience working with elderly population Preferred
