PURPOSE AND SCOPE: Under supervision is responsible for the operational condition, maintenance, and repair of all water systems, medical equipment, and mechanical/electrical systems so as to ensure safety of the patients as well as the staff operating the equipment. Responsible for inventory and housekeeping as required. Assists in cost containment in the facility. Supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. DUTIES / ACTIVITIES: CUSTOMER SERVICE: <ul><li>Responsible for driving the FMS culture through values and customer service standards.</li><li>Accountable for outstanding customer service to all external and internal customers.</li><li>Develops and maintains effective relationships through effective and timely communication.</li><li>Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.</li></ul> PRINCIPAL DUTIES AND RESPONSIBILITIES: MAINTENANCE/TECHNICAL:<ul><li>Maintains, tests and repairs all water treatment equipment, dialysis equipment, medical equipment, testing equipment, and mechanical/electrical systems as recommended by the manufacturer and by procedures established by FMCNA Clinical/Technical services as documented in the Technical Policies and Standards manual.</li><li>Documents all maintenance and repairs.</li><li>Ensures safety checks and routine tasks are conducted and documented in a timely fashion according to the established procedures.</li><li>Conducts housekeeping check and follows-up housekeeping problems.</li><li>Responsible for water treatment equipment cleaning and disinfecting.</li><li>Ensures all testing equipment is properly maintained and calibrated.</li><li>Ensures all areas are safe and clean - that environmental integrity and aesthetics are maintained.</li><li>Conducts building maintenance as needed.</li><li>Responsible for coordination of medical waste disposal.</li><li>Collects and organizes supplies restocking relevant areas as needed.</li><li>Assists in ordering and receiving disposable supplies.</li><li>Assists in creating SAP purchase orders for supplies.</li><li>Assists in conducting monthly and quarterly inventory of supplies with cost reporting through facility specific records.</li><li>Evaluates and reports the current equipment inventory to appropriate sources.</li></ul> PATIENT CARE: <ul><li>Ensures visibility to patients with daily walk through during treatment times, when possible.</li><li>Reinforces the concept of UltraCare&trade; and customer service - ensures availability for patient's questions and concerns.</li><li>As requested, provides instructive tours of the water treatment equipment for patients and explains the basics of the machines.</li></ul> CLERICAL & ADMINISTRATIVE: <ul><li>Processes water and dialysate samples according to specified procedures, package samples for shipping.</li><li>Participates in the completion and documentation of ongoing technical QAPI activities as appropriate.</li></ul> OTHER: <ul><li>Provides assistance at other facilities and/or acute programs, transport equipment and supplies as needed.</li><li>Other duties as assigned.</li></ul> PHYSICAL DEMANDS AND WORKING CONDITIONS: <ul><li>The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. PURPOSE AND SCOPE: Explores, recommends, and coordinates the insurance and potential financial assistance options available to kidney dialysis patients in a specified geographic area, while maximizing revenue for the company. Supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and company policy requirements PRINCIPAL DUTIES AND RESPONSIBILITIES: Meets regularly with dialysis patients at the clinic(s) in the assigned region to educate and coordinate insurance options: <ul><li>Educates on the availability of alternative insurance options (i.e., Medicare, Medicaid, Medicare Supplement, State Renal programs and COBRA).</li><li>Ensures patients have followed through with the application process.</li><li>Obtains premium statements and signatures from patients.</li><li>Discusses situation and options if employment status changes or other situations change.</li><li>Completes and follows up with paperwork when claims are disputed for non-payment. </li><li>Collects necessary documents to completed initial and annual indigent waivers.</li><li>Discusses insurance options when insurance contracts are terminated.</li></ul>Responsibilities involving Medicare and Medicaid include but are not limited to:<ul><li>Determining Medicare eligibility by meeting with the patients and contacting local Social Security offices to verify eligibility.</li><li>Discussing the Medicare application with eligible patients and assisting with the application process.</li><li>Acting as liaison between the patient and the local agents for Medicare terminations and re-in statements.</li><li>Completing the annual open enrollment and Medicare reinstatement papers with the patients.</li><li>Tracking 30-month coordinator period each month for those patients on employer Group Health Plans to ensure Medicare will be in place once coordination ends. </li><li>Monitoring and verifying the Medicaid status of each patient on a monthly basis and determining the spend down amounts</li><li>Works with patients to evaluate personal financial information and make determination for indigent program.</li><li>Completes initial Indigent waiver applications.</li><li>Tracks and completes annual indigent waiver applications.</li><li>Monitors all patients' insurance information to ensure that it is updated and accurate for the Accounts Receivable Department.</li><li>Addresses any identified anomalies or discrepancies, researches and answers questions as needed.</li><li>Meets with patients receiving direct payments from insurance companies to ensure payment of dialysis treatments owed to Fresenius.</li><li>Prepares, analyzes and reviews monthly reports to track work progress on caseloads; Analyzes patient reports from billing systems as an audit check to ensure the correct insurance information is entered into the billing system and that other changes are not overlooked. Researches and corrects any discrepancies identified.</li><li>Provides QA team members with monthly information regarding the details of the patients' primary and secondary insurance status as well as documentation regarding the plans of actions currently in place on a monthly basis as required by QA processes </li><li>Completes monthly audit exam to stay current on internal policies.</li><li>May present on insurance and financial assistance options to patients as necessary.</li><li> Assist with various projects as assigned by direct supervisor. </li><li>Other duties as assigned.</li></ul> Additional responsibilities may include focus on one or more departments or locations. PURPOSE AND SCOPE: Functions as part of the Home Therapies dialysis health care team. Ensures provision of quality home patient care in accordance with company policies, procedures and training. Supports the company's commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction. Actively participates in process improvement activities that enhance the likelihood that patients will achieve the company's Quality Enhancement Goals (QEP). PRINCIPAL DUTIES AND RESPONSIBILITIES: Staff Related: <ul><li>Provides safe and effective training and oversight of the delivery of home dialysis therapies to patients in compliance with standards outlined in the facility policy and procedure manuals, as well as regulations set forth by the Corporation, state and federal agencies.</li><li>Rotates with other licensed staff to ensure adequate coverage.</li><li>Participates in staff training and orientation of new staff as assigned.</li><li>Participates in all required staff meetings as scheduled.</li></ul>Quality: <ul><li>Provides outstanding quality of patient care, as defined by company quality goals and standards.</li><li>Submits CQI information to Clinical Manager for completion of HT QAI Template for inclusion in facility CQI packet.</li><li>Participates in all monthly CQI meetings.</li></ul>Education: <ul><li>Assists in the identification and evaluation of candidates for the company Home Therapies Program, including assessment of patients' home environment.</li><li>Trains home patients and their families/significant others on the safe and effective operation and maintenance (as appropriate), of all home therapies equipment through a formal Home Therapies training program.</li><li>Educates patients and family regarding End Stage Renal Disease (ESRD) and the delivery of the company Kidney Options Program.</li><li>Assesses patient and family readiness and ability to perform dialysis treatments at home and patients' home environment for suitability in the ongoing delivery of home dialysis therapy.</li><li>Provides ongoing education to patients regarding their renal dialysis, peritoneal or vascular access and home dialysis therapy, and other related health conditions.</li></ul>Dialysis Treatment: <ul><li>Provides safe and effective delivery of home therapy support care to patients with ESRD.</li><li>Assesses patients' response to home dialysis treatment therapy by following predetermined protocols.</li><li>Communicates patient related issues to the physician as needed.</li><li>Reports adverse patient events at home or in the clinic as well as equipment and technology related problems to the Clinical/Home Therapies management and physician including the correct documentation of such events.</li><li>Coordinates the transfer of patients to the hemodialysis clinic when needed and appropriate.</li></ul>Laboratory Related: <ul><li>Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System.</li><li>Follows company policies and government regulations related to the collection, packaging, and delivery of lab specimens and reporting of results.</li></ul>General Duties: <ul><li>Maintains competency with all emergency operational procedures, and initiate CPR and emergency measures in the event of a cardiac and/or respiratory arrest when the patient is in the medical office.</li><li>Administers medications as prescribed including following algorithms (as appropriate), and document appropriate medical justification if indicated and maintain records on controlled substances as required by law.</li></ul>Maintenance/Technical: <ul><li>Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.</li><li>Directs patients in the preventative maintenance of home therapies equipment, including water systems and analyses as appropriate.</li><li>Coordinates with the company products division on the delivery, operation and maintenance of all home therapy dialysis equipment.</li></ul>Medical Records & Documentation: <ul><li>Admits all new patients and documents as needed.</li><li>Documents all relevant data including physician orders, lab results, vital signs, and treatment parameters, and patient status and any treatment related data appropriately, and enters into Medical Information System.</li><li>Participates in and ensures all company policies regarding patient admission, transfer, and discharge are appropriately implemented.</li><li>Completes all monthly and annual reporting as required by company policies and government regulations.</li></ul>Other: <ul><li>Performs other related duties as assigned</li></ul> PHYSICAL DEMANDS AND WORKING CONDITIONS : The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. SUPERVISION: Assigned oversight of Patient Care Technicians/LPNs/LVNs/RNs as a designated Nurse in Charge, after meeting all the following:<ul><li>Successful completion of all FKC education and modality specific training requirements.</li><li>6 months experience in acute dialysis as a RN.</li></ul> EDUCATION AND LICENSURE: <ul><li>Current appropriate state licensure.</li><li>Current or successful completion of CPR BLS Certification</li><li>Must meet the practice requirements in all states employed.</li></ul> EXPERIENCE AND REQUIRED SKILLS :<ul><li>Entry level for RNs with minimum 2 years or more of Nephrology Nursing experience in the last 2 years as a RN.</li><li>Acute dialysis experience (preferred).</li></ul>The rate of pay for this position will depend on the successful candidate's work location and qualifications, including relevant education, work experience, skills, and competencies.
