Hourly position which will work 6a-6p alternating MWF/TTS each week. This person will serve as point of contact for nephrologists with 3-4 different nephrology groups with patients at hospitals located in:<ul><li>Minneapolis</li><li>St Paul</li><li>Coon Rapids</li><li>Fridley</li><li>Waconia</li><li>Shakopee</li></ul>Busiest programs are in Minneapolis & St Paul and would work out of those locations frequently. This candidate must be highly organized, will communicate constantly with our team leads at each location, and with nephrologists (will utilize Grasshopper), at the end of the day will publish the treatment schedules for the next day (Excel skills needed). PURPOSE AND SCOPE: The Inpatient Services Charge Nurse is an additional responsibility" assigned to the Inpatient Services RN, for a limited period of time, and for specific additional duties, as assigned by the Program Manager. PRINCIPAL DUTIES AND RESPONSIBILITIES: Under limited supervision, applies considerable complex knowledge to perform a broad range of tasks which include but are not limited to the following: <ul><li> Acts as a subject matter expert and resource for staff members on assigned Inpatient Center treatment floor. </li><li> Normally receives no instruction on routine daily work, and general instructions on newly introduced assignment, but may escalate issues to Program Manager for resolution, as deemed necessary. </li><li> Coordinate and schedule direct patient care staff to provide coverage of Acute Dialysis treatments in contracted hospitals/facilities and ensure efficient patient scheduling and staff utilization in accordance with the TAP scheduling tool. </li><li> Maintain and review the daily TAP tool for all hospitals/facilities under their direct supervision and submit results to the Program Manager for review. </li><li> Ensure all Acute treatments are performed in contracted hospitals/facilities according to the established policies and procedures, and in compliance, with all applicable regulatory requirement. Ensure accurate documentation of information related to patient treatment is entered into ACES during the time of treatment. </li><li> Monitor on a daily basis ACES compliance. </li><li> Perform Quality Assessment and Improvement (QAI) Program audits as directed to ensure the provision of quality patient care and compliance with all Joint Commission requirements. </li><li> Participate in QAI meetings as requested by the Program Manager to assure proper reporting of QAI activities </li><li> Assess daily patient care needs and communicate concerns to attending physician. Implement changes in patient care/treatment as directed and in collaboration with prescribing physicians. </li><li> Perform Acute treatments according to current Charge Nurse Guidance recommendations and oversee all dialysis related medical and emergency functions. </li><li> Communicates with the physician and other members of the healthcare team to interpret, adjust, triage and complete patient care plans in the most efficient and safe manner. </li><li> Coordinate with appropriate technical staff to ensure preventative maintenance program for routine machine maintenance, to ensure the timely repair of disabled machines, to coordinate infection control measures, routine water culturing, water testing, logging of results, transportation of equipment and supplies, and communicate technical information to the staff. </li><li> Serve as liaison between the administrative staff of each hospital/facility and the Program Manager. </li><li> Practice cost containment strategies in all areas including medical supply inventory and personnel. </li><li> Reviews and comply with the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations. </li><li>Assist with various projects as assigned by the Program Manager.</li><li> Other duties as assigned. </li></ul> PHYSICAL DEMANDS AND WORKING CONDITIONS :<ul><li> The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Acute Inpatient Charge Registered Nurse - RN - Dialysis

PURPOSE AND SCOPE: Supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FVC/FMCNA Compliance Program, including following all regulatory and FVC/FMS policy requirements. Provides direct nursing care pre, during and post procedure, under the direction of the Facility Manager and/or Charge Registered Nurse, utilizing standard nursing techniques to assist the physician in optimum patient outcomes following facility policies and procedures. PRINCIPAL DUTIES AND RESPONSIBILITIES: QUALITY: <ul><li> Responsible for supporting and driving FVC quality standards through meeting regulatory and accrediting body requirements and the practice of Continuous Quality Improvement (CQI), including use of FVC/FMS CQI tools. </li><li> Assists with implementation of FVC quality goals and facility specific action plans in order to achieve FVC quality standards. </li><li> Accountable for outstanding quality of patient care, as defined by the FVC quality goals, to ensure that FVC/FMC policies and procedures are followed. </li><li> Responsible for addressing and acting on adverse events and action thresholds in collaboration with facility management. </li><li> Maintains integrity of medical records and other FVC administrative and operational records. </li><li> Complies with all data collection and auditing activities. </li><li> Maintains facility environmental integrity and safety. </li></ul> PATIENT: <ul><li> Obtains necessary medical history, discusses procedure and answers questions, and performs nursing assessment on the day of the procedure. </li><li> Provides education to patients and patient's family regarding post procedure care of vascular access. </li><li> Monitors patient condition before, during and after the procedure, especially patients having moderate sedation and contrast media injections for possible complications or reactions. </li><li> Acts as a resource for the patient to address patient concerns and questions. </li><li> Identifies and communicates patient related issues to facility management, as needed. </li><li> Completes documentation of patient information from admission to discharge in an accurate manner. </li><li> Confirm written orders, consents and lab work are completed and documented in accordance with facility policies. This includes appropriate preparation of lab requisitions; delivery to appropriate labs; reporting and forwarding of lab results to appropriate physician. </li><li> Maintains competency with all emergency operational procedures, and initiates CPR and emergency measures, as needed. </li><li> Administers medications and IV moderate sedation to patients as directed per physician's orders, and in compliance with FVC, company, federal, state and local regulations. </li><li> Acts as scrub technologist to assist the physician during procedure as needed; maintaining a sterile technique. </li><li> Assists with patient workflow by transporting patients as needed. </li><li> Performs point - of - care laboratory testing as directed per physician's orders and in compliance with FVC, company, federal, state and local regulations. </li><li> Performs IV insertion as per physician orders. </li><li> Performs additional tasks as designated by Physician, practicing in compliance with state regulations. </li></ul> PHYSICIANS: <ul><li> Provides regular and effective communication with physicians on an as needed basis. </li><li> Attends and participates in CQI meetings with physicians, reporting on assigned topics. </li><li> May assist facility management in working with the physicians and staff to verify all necessary instruments and equipment. </li><li> Assists the physician in completing and reporting state required adverse event reports and reporting internally as per FVC/FMC policy and procedure. </li></ul> MAINTENANCE/TECHNICAL: <ul><li> Operates appropriate facility equipment and technology, including but not limited to, patient monitors, defibrillators, medical recording devices, sterilizers and computers in a safe and efficient manner. </li><li> Inspects operation of all facility equipment and reports malfunctions, and maintenance needs to ensure the safe and effective operation of all vascular access equipment. </li><li> Inspects operation of all facility operating systems, including temperature checks in appropriate areas and physical structure of the facility and reports any issues to the Facility Manager. </li><li> Ensures a clean, safe and sanitary environment in the vascular access facility at all times. </li><li> Ensures all blood spills are immediately addressed according to the FVC/FMCNA Bloodborne Pathogen Control Policies. </li></ul> ADMINISTRATIVE: <ul><li> Assists with efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the FVC/FMS formularies. </li><li> May assist in the development and revision of any applicable policies and procedures related to the facility to ensure continuous process improvement. </li><li> Assists with Continuous Quality Improvement (CQI) activities including monthly reports and attending meetings as required. </li><li> Assists with maintaining medication supply, including inventory control, checking for expired medications, and ensuring appropriate recording of controlled substances as required by law. </li><li> Completes usage logs for all medications with appropriate documentation, as well as, records of drug disposal in accordance with local, state, and federal law. </li><li> Assists with maintaining facility supply inventory, including ordering and restocking of supplies as needed. </li><li> Examines emergency equipment and supplies, including stocking of the crash cart on a regular basis. </li><li> Completes medical records in an accurate manner in compliance with FVC policies and procedures prior to discharge of the patient. PRINCIPAL RESPONSIBILITIES AND DUTIES CLINIC OPERATIONS: <ul><li>Manages the operations of the clinic, including costs, processes, staffing, and quality standards.</li><li>Provides leadership, coaching, and development plans for all direct reports.</li><li>Partners with internal Human Resources, Quality, and Technical Services departments.</li><li>Collaborates with or functions as the Home Therapies Program Manager to oversee the facility's Home Therapies Program.</li><li>Maintains integrity of medical and operations records and complies with all data collections and auditing activities.</li><li>Accountable for completion of the Annual Standing Order Review and Internal Classification of Disease (ICD) coding.</li><li>Manages clinic financials including efficient utilization of supplies or equipment and regular profits and loss review.</li><li>Responsible for all required network reporting and on-site state or federal surveys.</li></ul> PATIENT CARE: <ul><li>Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge of the patient.</li><li>Ensure education of the patient regarding quality measures, transplant options, modality awareness, and access care.</li><li>Acts as a resource for the patient and family to address concerns and questions.</li><li>Accountable for timely completion of patient care assessments and care plans.</li><li>Manages timely patient schedules to ensure facility efficiency and develops action plans for missed treatments.</li><li>Plans, coordinates, and validates patient eligibility for treatment.</li><li>Reports changes in patient status regarding any clinical, insurance, or travel and transportation issues. </li></ul> STAFF: <ul><li>Participates in the selection and hiring of new team members and ensures completion of new hiring orientation and mandatory trainings.</li><li>Responsible for all patient care employees completing appropriate training courses and that licensures/certifications are current.</li><li>Provides support for all clinical staff members at regular intervals and encourages professional growth.</li><li>Maintains current knowledge regarding company benefits, policies, procedures, and processes.</li><li>Completes employee evaluations and establishes annual goals.</li><li>Documents any disciplinary actions and confers with the clinic Director of Operations and Human Resources when needed.</li><li>Manages staff scheduling and payroll.</li></ul> PHYSICIANS: <ul><li>Facilitates the application process for physician privileges and compliance with Fresenius Medical Care Medical Staff By-Laws.</li><li>Responsible for strong physician relationships and ensures regular and effective communication.</li><li>Participates in Governing Body, an interdisciplinary team for each region including MSWs, Dietitians, MDs, DOs etc. that governs policies.</li></ul>EDUCATION AND REQUIRED CREDENTIALS:<ul><li>Bachelor's Degree or an equivalent combination of education and experience.</li><li>Graduate of an accredited School of Nursing (RN).</li><li>Current appropriate state licensure.</li></ul>EXPERIENCE AND SKILLS: <ul><li>Required:<ul><li>6+ years business operations experience in a healthcare facility.</li><li>12 months experience in clinical nursing.</li><li>6 months chronic or acute dialysis nursing experience.</li><li>Successfully pass the Ishihara Color Blind Test.</li></ul></li><li>Preferred but not required:<ul><li>3+ years supervisory or project/program management experience.</li><li>Med/surg or ICU/CCU experience.</li></ul></li></ul>PHYSICAL DEMANDS AND WORKING CONDITIONS:<ul><li>Day-to-day work includes desk and personal computer work and interaction with patients, facility staff, and physicians</li><li>Position may require travel between assigned facilities and various locations within the community and/or travel to regional, business unit, and corporate meetings may be required.</li><li>The work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels.</li><li>May be exposed to infectious and contagious diseases/materials.</li><li>Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. </li><li>The position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation.
