Description
Performs functions accurately in electronic systems to merge duplicate numbers by transferring cases, updating demographic information as appropriate, etc. Performs chart correction functions accurately to ensure appropriate documentation posts to correct patient/encounter and corrects identifiers that do not correspond with the remaining systems. Performs merge functions accurately in systems that need to be updated due to a duplicate medical record number inappropriate order and verifications. Validates if suspected or reported duplicate is a true duplicate. Combines duplicate medical record numbers accurately and appropriately by verifying and comparing the following information: a. Patient Name and spelling b. Birthdate c. Social security number d. Address e. Phone numbers f. Driver’s license number g. Signatures h. Names of relatives, friends, or emergency contacts i. Insurance plans and numbers j. Similarities in clinical information or physicians k. Any other pertinent information that may help identify if number is a duplicate Makes the appropriate determination of which medical record number will be kept by consideration of such things as: a. Number of cases attached to each medical record number b. Oldest dates on cases c. Outstanding charges on cases d. Performs functions accurately in order to electronically merge duplicate numbers by correctly looking at all number assignments, person numbers vs medical record numbers vs physician numbers vs employee numbers, etc. e. Demographic information f. Encounters g. Media type h. Charges i. Performs all functions associated with un-combining of incorrectly merged data. Conducts qualitative review and resolution of Identity Reports Patient Overlay: Review and resolves all updates to a patient’s name, DOB, and/or SSN a. Potential Merge: Evaluates and resolves duplicates flagged as “Marked for Merge” b. Potential Unmerge: Evaluates completed duplicates that may have been done in error. c. Duplicates Patients Report: Evaluates and resolves potential duplicates as determined by LLEAP d. Monitors origination of duplicate numbers to enable the appropriate feedback to originating areas. Updates ancillary departments with surviving medical record number information as appropriate. Performs assignment downtime of block numbers. Acts as a resource person to outside areas in making determination of identity. Assigns correct medical record number to documentation received in the department that has not been correctly identified. Answers phones in the number control area and acts as resource in correct identification of medical record numbers. Consistently manages time effectively.
