If you're passionate about community work, we invite you to join our team as a Community Health Worker, where you will play a key role in the Homeless Healthcare Collaborative team by promoting greater health equity and clinical outcomes for people experiencing homelessness. Primary duties will include:
• Help patients navigate and access community services, resources, and adopt healthy behaviors.
• Support providers and team members with care management and community outreach.
• Engage with homeless individuals in encampments, shelters, interim housing sites, health fairs, and other locations.
• Promote, maintain, and improve the health of unhoused patients.
• Provide social support and informal counseling to homeless individuals.
• Advocate for the health needs of homeless communities.
• Share the responsibility of driving the mobile clinic van.

Note: Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.

Salary Range: $23.90/hour - $44.59/hour

Job Qualifications
Qualifications

Required:
• Required to hold valid driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program.
• Basic Life Support (BLS) Certification from the American Red Cross or American Heart Association.

Preferred:
• Lived experience of homelessness.
• Fluency in Spanish.
• Experience navigating or helping people navigate medical clinic, medical insurance, and social services.
