Job Title: Case Manager
Job Description
The Case Manager is responsible for identifying complex cases based on enrollment criteria and managing a caseload of assigned members. The role requires performing routine follow-ups with patients, which may take place via phone, at the primary care provider’s office, in the patient's home, or other appropriate settings. The Case Manager will contact members after hospital discharge and ensure thorough documentation of interactions and outcomes within established timeframes.
Responsibilities
- Identify complex cases based on enrollment criteria.
- Monitor and manage assigned member caseloads.
- Perform routine follow-ups with each patient in various settings.
- Contact members post-hospital discharge and after transitional nurse visits.
- Discuss program details with members and initiate the enrollment process.
- Ensure documentation is thorough and completed within established timeframes.
- Document actions taken to prevent readmission and other transitions of care.
- Complete HRA required outreach within 30 days of eligibility or upload from health plan.
- Develop and update an individualized plan of care with S.M.A.R.T. goals.
- Collaborate with healthcare providers and staff to optimize care continuity.
- Educate members about available organizational and community resources.
- Mail out welcome packets including Resource Booklet information to members.
- Obtain updated orders from healthcare providers to initiate services.
- Submit orders with supporting documentation to the case management support team.
- Attend Care Coordination IDT meetings and staff meetings.
- Collaborate with the scheduling team for appropriate member scheduling.
- Assist with processing referrals to expedite care and optimize health.
- Submit communication forms and advanced directives according to departmental policies.
- Stay updated on hospital discharge planning intervention expectations.
Essential Skills
- Acute care experience.
- Registered Nurse (RN) or Charge Nurse experience.
- Proficiency in case management and hospital settings.
- Graduate of an accredited Licensed Vocational Nursing program.
- Current LVN nursing license.
- 2 years of clinical nursing experience or 1 year of case management experience.
- Knowledge of medical terminology.
- Proficiency in Microsoft Office applications.
Additional Skills & Qualifications
- Ability to utilize computers and software applications effectively.
- Strong communication and organizational skills.
Work Environment
The position is a fully onsite role in an office environment, operating Monday through Friday from 8am to 5pm. This is a minimum 12-week assignment covering a leave of absence in the department. The dress code is scrubs.
Job Type & LocationThis is a Contract position based out of Victorville, CA.
Pay and BenefitsThe pay range for this position is $24.19 - $31.45/hr.
