Job Description
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. This role focuses on providing routine nursing care and offers support in managing high-risk and chronic care, along with referral specialist support. Our healthcare facility is a Federally Qualified Health Center, providing primary healthcare services to children and adults in Greenwich and Stamford, with a particular focus on populations residing in or near public housing.
Responsibilities
- Provide routine nursing care to patients in a primary care setting.
- Support high-risk and chronic care management, including coordinating care for elderly and disabled patients.
- Manage care for patients with chronic conditions such as Diabetes, HIV, Hypertension, and Obesity.
- Assist with referral processes and care coordination.
- Travel between Stamford and Greenwich as required for patient care.
- Support women's health initiatives and conduct medical assessments.
- Monitor and record vital signs accurately.
Essential Skills
- Current LPN licensure in Connecticut.
- Minimum of 3 years of experience working as an LPN.
- Experience in high-risk or chronic care management.
- Proficiency with electronic health records (EHR).
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Bilingual proficiency in Spanish or Haitian Creole.
Additional Skills & Qualifications
- Minimum of 2 years of experience in a primary care or outpatient setting preferred.
- Knowledge of population health, care coordination, and referral processes.
- Strong clinical skills in primary care and chronic disease management.
- High attention to detail and ability to manage multiple responsibilities.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Compassionate, patient-centered approach to care.
Work Environment
The work environment is a community health center with flexible scheduling. The office operates Monday through Friday from 8 am to 5 pm, and caters to patients of all ages, from infants to geriatrics. Training will be provided, ranging from 2 to 4 weeks, depending on the candidate’s ability to learn quickly. Training includes onboarding, orientation, and shadowing. Physical requirements include the ability to stand and walk for up to 8 hours a day, stoop, kneel, crouch, extend without restrictions, and lift a minimum of 50 lbs. Excellent hand/eye coordination is necessary.
