Job Description
We are seeking a compassionate and skilled Licensed Medical Social Worker to join our Behavioral Health team in Maui. This role is essential in supporting patients with mental health and psychosocial challenges, ensuring they receive holistic, culturally sensitive care. The ideal candidate will be experienced in behavioral health, crisis intervention, and care coordination within a multidisciplinary healthcare setting.
Responsibilities
- Conduct comprehensive psychosocial assessments for patients with behavioral health needs.
- Develop and implement individualized care plans in collaboration with medical and mental health professionals.
- Provide short-term counseling, crisis intervention, and support to patients and families.
- Facilitate referrals to community resources, including housing, substance use treatment, and financial assistance.
- Participate in interdisciplinary team meetings to coordinate care and advocate for patient needs.
- Maintain accurate documentation in compliance with HIPAA and organizational standards.
- Support discharge planning and continuity of care, especially for high-risk patients.
- Promote cultural competence and sensitivity in all interactions, respecting the diverse communities of Maui.
Essential Skills
- Licensed Clinical Social Worker (LCSW) in the state of Hawaii.
- Multiple years of experience within behavioral health social work.
- Experience in hospital settings relating to discharge planning and case management.
- Strong interpersonal, communication, and organizational skills.
- Ability to work independently and as part of a collaborative team.
Additional Skills & Qualifications
- Master’s Degree in Social Work (MSW) from an accredited institution.
- Current Hawaii licensure as a Licensed Social Worker (LSW), Licensed Clinical Social Worker, or Licensed Marriage and Family Therapist.
- Minimum 2 years of experience in a behavioral health or medical setting preferred.
- Knowledge of Hawaii’s healthcare system and local community resources is highly desirable.
