Job Description
We are seeking a dedicated Nurse Supervisor to oversee and document healthcare services, medical needs, and interventions for our program members. This role includes participation in Individual Support Plans and case conferences. The Nurse Supervisor will advocate for the optimal health of members and coordinate necessary medical services with Primary Care Physicians. The position also involves ensuring medical compliance, managing treatment plans, and maintaining confidential medical files.
Responsibilities
- Conduct preadmission reviews, member assessments, and develop AFC plans of care.
- Recruit, train, supervise, and support AFC caregivers, ensuring compliance with training and care standards.
- Perform quality assurance tasks, including audits and policy reviews.
- Report high-profile case issues.
- Collaborate with other agencies and maintain professional relationships to promote member care and services.
- Maintain certifications, licensure, and compliance with all safety, confidentiality, and regulatory standards.
- Patient care expertise.
- Triage and medication adherence skills.
- Willingness to travel and supervisory experience.
- Experience with either AFC or the relevant field (both preferred).
- Proficiency in Electronic Health Records.
- Current RN licensure by the Massachusetts Board of Registration in Nursing.
- 2+ years recent experience in direct care of the elderly or disabled adults.
- Minimum of one-year supervisory experience preferred.
- Experience in AFC (Home Health) and/or relevant field.
- EMR Experience, particularly with Icentrix.
- Ability to train adult learners.
- Basic computer skills and working knowledge of remote communication tools such as Zoom.
- Valid driver’s license, availability of a private vehicle, up-to-date car insurance, and proof of annual vehicle inspection.
- Satisfactory CORI, SORI, and National Fingerprint Check.
This role offers a work-life balance with schedule flexibility and the potential to work from home once tenured. The typical work hours are Monday to Friday, 8:30am to 4:30pm, with flexibility to stay late as needed.
