Job Description
We are seeking a compassionate and dedicated pharmacist to provide care for Medicaid patients in Missouri through telehealth and phone communication. This role involves acting as a pharmacist case manager, supporting patients by building trust and rapport, coordinating with pharmacies and providers, and developing treatment plans.
Responsibilities
- Provide Medication Therapy Management (MTM) services to Medicaid patients, including counseling on medication adherence and providing medication and self-care education.
- Coordinate diagnostic testing and medication therapy changes with physicians.
- Connect participants with community-based resources as needed.
- Assess participants' health status and develop medication treatment plans.
- Monitor and evaluate participants' responses to therapy.
- Conduct comprehensive medication reviews to identify and resolve medication-related problems.
- Document care provided and communicate essential information to primary care providers.
- Provide education and training to enhance understanding and appropriate use of medications.
- Coordinate transportation services for in-person provider meetings.
- Maintain proper documentation of each service provided, including patient information, time spent, and treatment plans.
- Coordinate patient signatures and collaborate with providers, community health workers, case managers, and family members.
- Current Pharmacist License in Missouri
- Experience in MTM and ambulatory settings
- Strong communication skills, especially over the phone
- Ability to write SOAP notes
- Comfortable using phone, email, and fax for communication
- Enrollment in the state's Medicaid program (if not already, must be completed prior to starting at no cost)
- Ideal for new graduates or those with experience in insurance settings
- Personable and compassionate, with a passion for helping people with limited income and resources
This is a fully remote position where you can create your own work schedule, ideally between the hours of 6 am to 7 pm. Pharmacists are responsible for scheduling 8 hours of patient contact per day, with additional time for documentation. You will be part of a large initiative supporting over 30-50 thousand patients, working in a culture of compassionate pharmacists committed to providing accessible, ongoing care.
