Job Title: Registered Nurse
Job Description
As a Registered Nurse, you will ensure the quality and safe delivery of nursing services in the Diagnostic & Treatment Center (DTC) and Day Health Center (DHC). You will assess, plan, implement, and coordinate nursing care in the Medical Practice and DHC.
Responsibilities
- Investigate and assist in documenting and resolving customer issues and grievances in a timely, accurate, and courteous manner, including working with other departments to resolve the source of the problem(s).
- Evaluate and promote a safe environment for participants.
- Document all new medications administered at the DTC in the Participant’s electronic medical record as per MD’s orders.
- Identify and partner with other departments to resolve complex customer service inquiries.
- Assist with the coordination/preparation of mail order prescriptions. Complete prescription log and prepare mailing list.
- Coordinate and collaborate with the Site Medical Director and members of the interdisciplinary team (IDT) regarding Infection Control and Surveillance activities at the PACE Center.
- Provide education to participants regarding all PACE services.
- Respond to email messages from vendors, participants, or providers seeking basic information or a response from a staff member.
- Address site issues as they occur.

Additional Skills & Qualifications
- Minimum of one (1) year general clinical experience in a certified home health agency, acute care, medical surgical, and/or critical care.
- Geriatric experience required.
- Customer Service experience required.
- Minimum of one (1) year of experience working with an elderly population, or appropriate training from the PACE organization if less than one year.
- Basic Life Support certified by the American Heart Association or American Red Cross.
- Self-directed with the ability to work with or without supervision.
- Proficient in computer use for email, word processing, research, and maintenance of Participant records.
- Demonstrated ability to establish good rapport with participants and peers.
- Tactful, courteous, and pleasant telephone manner.
- Excellent written and verbal communication skills.
- Professional attitude.
- Must be able to work weekends.
Work Environment
The position is based in a Community Health Center within the New York metropolitan area, including locations in the Bronx and Brooklyn. The schedule is Monday to Friday, 8:30 am to 5:00 pm, with a one-hour lunch break. The work environment is collaborative, requiring coordination with various departments and teams to provide comprehensive care and services to participants.
