Job Description

This role in care management focuses on transitions of care, including coordination and medication reconciliation, aiming to reduce emergency department visits and readmissions. The position involves handling all inpatient discharges, except labor and delivery and oncology follow-up, and addressing all emergency department visits, with particular emphasis on patients with chronic conditions.


Responsibilities

  • Coordinate transitions of care and ensure smooth patient discharge processes.
  • Conduct medication reconciliation to promote patient safety and reduce readmissions.
  • Focus on reducing emergency department visits and readmissions.
  • Manage inpatient discharges, excluding labor and delivery and oncology follow-up.
  • Address emergency department visits, especially for patients with chronic conditions.

Essential Skills

  • Valid RN license in Washington State.
  • 1+ years of experience in Acute Hospital, Long Term Acute Care, Rehab, or Skilled Nursing settings.
  • Proficiency in case management and care coordination.
  • Experience with EPIC software.
  • Utilization review and appeals handling capabilities.
  • Discharge planning and population health management.

Additional Skills & Qualifications

  • Basic Life Support (BLS) certification.

Work Environment

We have different types of shifts available such as 4x10s, 5x8s and 3x12s. 4x10 and 5x8 shifts are Monday-Friday from 7 am - 5:30 pm. The 3x12 shifts are day or night shift: 7 am - 730 pm or 730 pm - 7 am.
