Sr Director Managed Care and Payor Strategy PURPOSE AND SCOPE: The Senior Director of Managed Care & Payor Strategy is a strategic leader responsible for driving initiatives that strengthen relationships with Managed Care Payor Organizations and optimize contractual performance. Sr Director Managed Care and Payor Strategy

PURPOSE AND SCOPE: The Field Service Technician performs client-site installation, service and maintenance on company manufactured and/or distributed equipment within the assigned regional area(s). The incumbent provides technical support in the diagnosis of operational and/or technical issues of applicable equipment. Field Service Technician

PURPOSE AND SCOPE: The student intern supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. The student intern functions as part of the dialysis health care team in providing medical nutrition therapy for patients under the direct supervision of a registered dietitian in accordance with FMCNA policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. DUTIES / ACTIVITIES: Dietetic Intern: An individual who currently is enrolled in a Dietetic Internship or a university-based Coordinated Program in Dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND). The dietetic student/intern may under direct guidance of the Renal Dietitian:<ul><li>Review of Renal Nutrition Basics and other applicable components of the Orientation and Training requirements of staff dietitians</li><li> Review the typical workflow for the Renal Dietitian, and/or Role of the RD </li><li> Conduct a nutrition interview with a patient </li><li> Calculate a diet prescription recommendation </li><li> Provide nutrition counseling to patient/designated caregiver on the prescribed renal diet </li><li> Collect data for initial, 90 day, semi-annual (if applicable by State), and annual Comprehensive Interdisciplinary Assessments </li><li> Assist the registered dietitian in development and implementation of nutrition plan of care </li><li> Review monthly nutrition data with patients and/or designated caregivers </li><li> Practice writing monthly nutrition progress notes </li><li> Provide input to the development and implementation of nutrition plan of care </li><li> Create nutrition related projects for patients or staff (such as, bulletin board, patient/staff education handouts, games, lobby activity) </li><li> Attend Quality Assessment and Performance Improvement and /or other interdisciplinary team meetings with the registered dietitian, and participate as appropriate </li></ul> PHYSICAL DEMANDS AND WORKING CONDITIONS: The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. SUPERVISION: The Clinical Manager assigns and closely monitors all Dietetic Intern assignments with direct supervision from a Registered Dietitian based on the agreed education plan. Student Dietitian Clinical Placement

PURPOSE AND SCOPE: Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. Supports the FMCNA commitment to the Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMCNA Quality Goals. PRINCIPAL DUTIES AND RESPONSIBILITIES: <ul><li> Patient Assessment / Care Planning / Counseling <ul><li> As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment. </li><li> Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life. </li><li> Utilizes FKC patient education programs, established social work theory and methods, social work focused interventions, and quality of life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals. </li><li>Provides monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.</li><li>Provides supportive counseling services to patients as permitted within the scope of their clinical training and state license.</li><li> Provides educational and goal directed counseling to patients who are seeking transplant. </li><li> Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes. </li><li> Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education. </li><li> Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons. </li><li> In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care. </li><li> Knowledgeable of and adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation. </li><li> Documents based on MSW interaction and interventions provided to patient and/or family. </li></ul></li><li> Quality <ul><li> Provides psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level. </li><li> Participates in monthly Quality review meetings with the interdisciplinary team. Reports on quality indicators related to adherence, such as Missed and Shortened Treatments, Quality of Life Trends, and Service Recovery. </li></ul></li><li> Patient Education <ul><li> Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs. </li><li> With other members of the interdisciplinary team, provides appropriate information about all treatment modalities. </li><li> Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management. </li><li> Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available. </li><li> Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them. </li><li> Collaborates with the team on appropriate QAI activities. </li></ul></li><li> Patient Admission and Continuity of Care <ul><li> Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns. </li><li> Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment. </li><li>The Social Worker will interview the patient to identify root causes or concerns for the discharge request, (i.e. transfer to hospice, relocation, dissatisfaction with services or staff) and share causes/concerns with operational leadership,</li></ul></li><li> Insurance and Financial Assistance <ul><li> Collaborates and functions as a liaison for patient with Insurance Coordinators to address issues related to insurance. </li><li> In collaboration with Insurance Coordinators, provides information and education to patients about payment to dialysis (federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs). </li><li> Collaborates with the Insurance Coordinator of any changes to patient state that impacts insurance i.e. transplantation, discharged, loss of coverage, or extended travel. </li><li> Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills </li></ul></li><li> Staff Related <ul><li> Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager. </li><li> Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources). </li><li> Provides training to staff pertaining to psychosocial topics as needed. </li><li> Contributes and participates with weekly team huddles. Master Social Worker - MSW Part time

PURPOSE AND SCOPE: Supports FMCNA's mission vision core values and customer service philosophy. Adheres to the FMCNA Compliance Program including following all regulatory and FMS policy requirements. Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP). DUTIES / ACTIVITIES: CUSTOMER SERVICE: <ul><li> Responsible for driving the FMS culture through values and customer service standards. </li><li> Accountable for outstanding customer service to all external and internal customers. </li><li> Develops and maintains effective relationships through effective and timely communication. </li><li> Takes initiative and action to respond resolve and follow up regarding customer service issues with all customers in a timely manner. </li></ul> PRINCIPAL RESPONSIBILITIES AND DUTIES PATIENT CARE: <ul><li> Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC) Adequacy Monitoring Program (AMP) Urea Kinetic Modeling (UKM) and regarding disease process/access. </li><li> Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor. </li><li> Evaluate vascular access for patency and report any unusual findings to nurse supervisor. </li><li> Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor. </li><li> Perform vascular access evaluation pre-treatment and report unusual findings to nurse supervisor. </li><li> Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor. </li><li> Perform administration of Heparin as delegated or as allowed by state law. </li><li> Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD). </li><li> Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed. </li><li> Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor. </li><li> Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document. </li><li> Discontinue dialysis treatment according to established procedures </li><li> Evaluate patient prior to termination of venous access - standing & sitting blood pressure. </li><li> Obtain Hemostasis and apply appropriate dressings. </li><li> Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor. </li><li> Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification. </li><li> Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed. </li><li> Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160. </li><li> Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification. </li><li> Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes. </li><li> Report any significant information and/or change in patient condition directly to the nurse supervisor. </li><li> Observe patient and conduct machine safety checks according to facility policy. PURPOSE AND SCOPE: Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for assigned patients in accordance with FMCNA policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. PRINCIPAL DUTIES AND RESPONSIBILITIES:<ul><li>Provides patient care under the direct visual supervision of a Registered Nurse (FMCNA RN) or in a treatment room immediately adjacent to an RN. Responsible for reporting and documenting all new and unusual incidents, information, complaints, or problems to the RN.</li><li>Promotes and assists in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) Activities; including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP). </li><li> Promotes and assists in meeting Total Area Productivity goals (TAP).</li><li> With documented competency and proficiency may set up and prime equipment as directed by the Fresenius Nurse for renal replacement therapies (hemodialysis, continuous renal replacement and peritoneal dialysis therapies). </li><li> Welcomes assigned patients and inquires as to their well-being since their last treatment. Reports any complaints or observations to the FMCNA RN. </li><li> Evaluates vascular access for patency and reports any unusual findings to FMCNA RN. </li><li> Obtains necessary pre and post treatment vital signs and weight. Reports unusual findings to FMCNA RN. </li><li> (Stated above)Performs vascular access cannulation and reports any difficulties in cannulation or unusual findings to FMCNA RN. </li><li> Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed. </li><li> Responsible for ensuring appropriate safety alarms are enabled including narrow venous limits (NVL 160). </li><li> Records accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification. </li><li> Initiates renal replacement treatment according to prescribed orders including setting blood flow rate (QB) and dialysate flow rate (QD) or peritoneal dialysis as prescribed by the attending nephrologist. </li><li> Collects, labels specimens and ensures timely delivery to laboratory per facility policy. </li><li> Evaluates intradialytic problems and provides intervention as prescribed by physician's order or as directed by FMCNA RN. Monitors patient's response to dialysis therapy and report any unusual findings to FMCNA RN. </li><li> Obtains vital signs prior to reinfusion; performing all relevant functions necessary for the discontinuation of treatment- documentation. </li><li> Reports any change or unusual findings to FMCNA RN. </li><li> Performs and document any intervention for unusual patient status and document patients' response to intervention. </li><li> Evaluates patients' vascular access during treatment including arterial and venous monitoring pressures, provides appropriate intervention as needed, documents and reports any unusual findings to the FMCNA RN. </li><li> Discontinues dialysis treatment according to established procedures. </li><li> Evaluates patient prior to termination of venous access - standing and sitting blood pressures prior to termination of venous access. Obtains hemostasis and applies appropriate dressings. Evaluates the patient prior to discharge and reports any unusual findings to the FMCNA RN </li><li> Performs and records Pre and Post dialysis evaluation, weight and vital signs with initial identification. </li><li> Other duties as required. </li></ul> PHYSICAL DEMANDS AND WORKING CONDITIONS : <ul><li> The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Equipment aids and/or coworkers may provide assistance. </li><li> This position requires frequent, prolonged periods of standing and the employee must be able to bend over.
