Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests 
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms: 
Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care. 
Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures. 
Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs. 
Position Summary
<ol>
 <li>Knowledge of HDMC policy/procedure, nursing certification requirements for own and others positions. Performs own job duties and ensures that the supervised staff perform their duties within legal and professional regulations and guidelines. Ensures that self and those supervised apply nursing principles, techniques and practices according to HDMC policies and procedures. Leads by example to promote excellence in resident care. Models professional demeanor and acts as a mentor.</li>
 <li>Knowledge of Residents Bill of Rights and Elder Abuse reporting regulations. Observes Residents Bill of Rights and Reports incidents of elder abuse, if indicated. Ensures supervised staff also is aware of and follow these regulations. </li>
 <li>Arrives and leaves work timely. Follows HDMC Human Resources policies, especially regarding dressing code, call off policy, breaks, lateness and sleeping while on duty.</li>
 <li>Ensures staffing for own shift and the following are adequate and appropriate according to set staffing guidelines. Contacts other nursing departments before calling in or calling off department staff. Appropriately and fairly sets staff assignments for their shift, and the following shift. Ensures staff workload is reasonable and as equally divided as possible. </li>
 <li>Functions as the primary resource at the assigned unit, manages patient flow, residents’ complaints, employee concerns, and is the primary contact for all physicians within the facility. </li>
 <li>Answers facility phone calls timely, responds to call lights via the call light system and delegates when appropriate. </li>
 <li>Demonstrates ability to delegate and achieve results through others.</li>
 <li>Demonstrates understanding of and ensures compliance with all State and Federal requirements.</li>
 <li>Assumes leadership role for daily operations and takes appropriate action. </li>
 <li>Rounds on residents, nursing staff, and families daily to ensure good outcomes. </li>
 <li>Supervises nursing staff, assists with hiring, education and disciplinary action of nursing staff. </li>
 <li>Assists with writing and revising, and ensures staff understanding and compliance with policies, procedures and standards.</li>
 <li>Demonstrates the ability to coordinate response to emergencies in a calm and efficient manner.</li>
 <li>Understands and demonstrates ability to work within the departmental budget. </li>
 <li>Ensures staff schedule is accurate and timely, and checks staffing needs daily to provide that minimum ppd hour are met.</li>
 <li>Meets regularly with the medical director and other staff physicians to ensure proper communication is maintained.</li>
 <li>Ensures that communication with the Acute Hospital is accurate and cooperative.</li>
 <li>Functions as a floor nurse, and/or certified nursing assistant when the facility needs so warrant. </li>
 <li>Completes all daily, weekly, and monthly assignments as assigned. </li>
 <li>Completes other duties as assigned. </li>
 <li>Meets at least weekly with the DON. </li>

Minimum Education/Certification 
<ul>
 <li>Associate Degree</li>
 <li>Current California Registered Nurse License (RN)</li>
 <li>BLS Certification</li>
</ul>
Minimum Experience
<ul>
 <li>2 years experience as an RN/LVN</li>
 <li>Previous Subacute/ventilator experience preferred</li>

 Position Summary
Evaluates patients regarding the application of a wide variety of therapeutic techniques fro rehabilitation of speech, language, hearing, and oral motor disorders.
 
Primary Duties 
<ul>
 <li>Maintains established department policies, procedures, objectives, quality improvement, safety, environmental and infection control standards.</li>
 <li>Gathers data related to patient’s communication disorder, selects appropriate evaluation tool(s) for the purpose of assessment, and administers diagnostic testing procedures to determine the extent of the patient’s impairment and the prognosis for improvement.</li>
 <li>Performs evaluations with special attention to receptive and expressive language skills, speech fluency, vocal and oral motor competence, articulation, auditory skills, cognitive linguistic skills and swallowing disorders.</li>
 <li>Develops and maintains long and short-term treatment goals for each patient. Re-evaluates and modifies treatment programs as indicated.</li>
 <li>Maintains active and support communication with the patient regarding progress, problems, home programs and other issues related to the therapeutic process.</li>
 <li>Discusses patient care programs with other specialists, including nurses and physicians. Notifies other specialists of recommendations regarding changing treatment programs or suggesting treatments not originally ordered by the physician.</li>
 <li>Serves as a resource to nursing and other disciplines.</li>
 <li>Prepares written documentation as required by the profession and the department.</li>
 <li>Enhances professional growth and development through participation in educational programs, current literature, workshops and in-service meetings.</li>
 <li>Demonstrates sensitivity and competence in the care of the ill and/or dying adolescent, adult, geriatric and pediatric patients relative to determining each patient’s age related physical, psychological and cultural needs.</li>

Minimum Education/Certification/Experience:

 <li>Master’s Degree in speech-language pathology</li>
 <li>2 Years of SLP experience</li>
 <li>Licensed as a Speech Pathologist in California</li>
 <li>California driver’s license and continuous insurance required</li>

Preferred Experience/Availability

 <li>Outpatient experience with preschool population and adults</li>
 <li>Availability to see adult patients in Acute/Skilled nursing environment</li>
