The Certified Nursing Assistant (CNA) cares for residents under the direction and supervision of a registered nurse or a licensed practical/vocational nurse. This role assists residents with activities of daily living (ADLs), ensures a safe and comfortable environment, and supports licensed nursing staff in delivering individualized care in accordance with regulatory requirements and facility policies.

RESPONSIBILITIES:
The essential functions of this position include the following. This list is not intended to be exclusive or exhaustive. There may be additional essential functions based on the unique nature of the position or environment.

<li>Provide direct resident care, including assistance with bathing, grooming, dressing, toileting, incontinent care as applicable, oral hygiene and feeding. </li>
<li>Measure and record vital signs, weight, percentage of meal consumed intake/output as directed. </li>
<li>Assist residents with mobility, transfers, and position using proper body mechanics and lift equipment as applicable. </li>
<li>Respond promptly to call lights, resident needs, and changes in condition; report observations to licensed nursing staff. </li>
<li>Support residents’ independence and dignity, while ensuring safety and comfort </li>
<li>Maintain accurate and timely documentation and in accordance with facility procedures </li>
<li>Follow infection control, safety, and universal precaution guidelines at all times. </li>
<li>Provide emotional support and companionship to residents, fostering a positive living environment </li>
<li>Maintains a clean, safe and comfortable environment </li>
<li>Provides postmortem care </li>

Education and/or Experience:

The LPN Supervisor is responsible for providing, overseeing and coordinating the delivery of resident care during assigned shifts in accordance with federal, state, and local regulations and facility policy under the direction and supervision of the Director of Nursing/designee.  This role provides clinical supervision to nursing assistants, ensures continuity of care, and serves as a key resource for residents, families, and staff.

The MDS Nurse is responsible for assisting with the accurate and timely completion of the Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), care planning and supporting documentation in accordance with federal and state regulations. This role ensures assessments reflect each resident’s clinical status and supports individualized care planning.
