Job Description

We are seeking an RN Case Manager to perform telephonic case management for Medicare Advantage members in the state of Florida. The case manager will be responsible for performing care coordination activities, developing care plans and assessments, conducting new referrals, calling members, and building new cases in Athena (EMR).

Responsibilities

• Perform telephonic case management for Medicare Advantage members.
• Conduct care coordination activities.
• Develop care plans and conduct assessments.
• Handle new referrals and call members.
• Build new cases in Athena (EMR).
• Manage a caseload of at least 73 patients.

Requirements

• RN license (FL license required).
• 2 years of case management experience from hospital, acute care, home health, or hospice.
• Experience with Athena EMR software/platform.
• Strong computer skills including Microsoft Suites, Excel, data entry, and computer documentation.

Additional Skills

• OASIS EMR experience.
• Prior experience in a fast-paced/high acuity nursing environment.
• Excellent patient care and triage skills.
• Strong customer service and healthcare knowledge.

Work Environment

• This hybrid opportunity with a schedule from Monday to Friday, 8:00 AM - 5:00 PM.
