Job Title: Inpatient Clinical Pharmacist


Job Description

We are seeking a dedicated and experienced Inpatient Clinical Pharmacist to provide pharmaceutical care to adult inpatients. The role involves ensuring safe and effective medication use across multiple departments, including Med/Surg/Telemetry, ICU, OR, SDS, Radiology/Interventional Radiology, and the ER. The pharmacist will also support the outpatient infusion department and oversee medication preparation and dispensing, adhering to safety protocols and hospital guidelines.


Responsibilities

  • Provide pharmaceutical care to adult inpatients across 54 Med/Surg/Telemetry beds and 10 ICU beds.
  • Collaborate with multidisciplinary teams in the Operating Room (OR), Same-Day Surgery (SDS), Radiology/Interventional Radiology (IR), and Emergency Room (ER).
  • Support the outpatient infusion department by preparing and managing chemotherapy and non-chemotherapy infusions.
  • Oversee medication preparation and dispensing for inpatient and Emergency Room patients, adhering to safety protocols and hospital guidelines.
  • Compound hazardous and non-hazardous medications in compliance with regulatory standards.
  • Utilize Pyxis for automated medication management and Epic for charting and documentation.
  • Leverage PharmacyOneSource for preferred pharmacy workflow management, if applicable.
  • Ensure compliance with institutional policies, state, and federal regulations.
  • Provide patient education and collaborate with healthcare providers to optimize therapeutic outcomes.

Additional Skills & Qualifications

  • 2+ years of inpatient experience
  • PharmD degree
  • NJ License

Work Environment

You will work in a dynamic hospital setting, utilizing advanced technologies such as Pyxis for automated medication management and Epic for charting and documentation. The role requires adherence to regulatory standards and institutional policies, ensuring a safe and compliant work environment. The facility supports a collaborative culture with multidisciplinary teams across various departments.
