Job Description
As a Licensed Practical Nurse (LPN) in the home care division, you will play a crucial role in delivering comprehensive healthcare services to patients. You will work one-on-one in a patient's home under the supervision of a registered nurse or physician, providing direct patient care, administering medications, and assisting with various healthcare procedures in accordance with established protocols and regulatory guidelines.
Responsibilities
- Conduct comprehensive assessments of patients' health status.
- Administer medications, treatments, and therapies as prescribed by the physician or registered nurse.
- Monitor patients' vital signs and report any significant changes or abnormalities to the appropriate healthcare provider.
- Assist patients with activities of daily living, including bathing, grooming, dressing, and toileting.
- Provide wound care, injections, and other skilled nursing services as needed.
- Educate patients and their families about health management techniques, medication administration, and disease prevention strategies.
- Maintain accurate and up-to-date patient records, including documentation of assessments, interventions, and outcomes.
- Collaborate with other members of the healthcare team to ensure continuity of care and optimal patient outcomes.
- Adhere to all regulatory requirements, policies, and procedures governing home healthcare services.
Essential Skills
- Valid New York State licensure as a Licensed Practical Nurse (LPN).
- Current CPR certification.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care delivery.

- Reliable transportation and willingness to travel to patients' homes within the designated service area.
