Licensed Practical Nurse (LPN) - Home Care Division

As a Licensed Practical Nurse (LPN) in the home care division, you will play a crucial role in delivering comprehensive healthcare services to patients.

You will work one-on-one in a patient's home under the supervision of a registered nurse or physician, providing direct patient care, administering medications, and assisting with various healthcare procedures in accordance with established protocols and regulatory guidelines.


Top 3 Must-Have Skills:

  1. Valid New York State licensure as a Licensed Practical Nurse (LPN): Ensuring compliance with state regulations and the ability to practice legally.
  2. Current CPR certification from AHA or RedCross: Essential for emergency situations and patient safety.
  3. Strong clinical assessment and critical thinking skills: Crucial for accurately evaluating patient conditions and making informed decisions.

Responsibilities:

  • Conduct comprehensive assessments of patients' health status.
  • Administer medications, treatments, and therapies as prescribed by the physician or registered nurse.
  • Monitor patients' vital signs and report any significant changes or abnormalities to the appropriate healthcare provider.
  • Assist patients with activities of daily living, including bathing, grooming, dressing, and toileting.
  • Provide wound care, injections, and other skilled nursing services as needed.
  • Educate patients and their families about health management techniques, medication administration, and disease prevention strategies.
  • Maintain accurate and up-to-date patient records, including documentation of assessments, interventions, and outcomes.
  • Collaborate with other members of the healthcare team to ensure continuity of care and optimal patient outcomes.
  • Adhere to all regulatory requirements, policies, and procedures governing home healthcare services.

Essential Skills:

  • Comfortable working alone with patients.
  • Willingness to work12-hour shiftsanda minimum of 3 days per week, withflexibility for weekdays and weekends.
  • Willingness to workat least an entire borough, preferablytwo, and ideally able to commute and work in multiple boroughs.
  • Willingness to work for a maximum of$40 per hour.
  • Reliable transportation and willingness to travel to patients' homes within the designated service area.

Soft Skills:

  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and patient-centered approach to care delivery.
  • Comfortable working with patients who have tracheostomies, ventilators, and feeding tubes, including entry-level candidates willing to learn and perform job duties associated with these patient conditions.

Work Environment: This position involves providing care in a home care setting, offering a personalized and patient-focused work environment. The role requires travel to patients' homes within the designated service area. Benefits include a competitive salary, health insurance, and paid time off.
