What You'll Do
- Conduct clinical medication reviews and make evidence-based therapy recommendations
- Provide patient counseling and education for specialty medications
- Collaborate with physicians, nurses, and care teams to support treatment success
- Verify prescriptions and ensure accuracy and compliance
- Educate patients on injectable therapies and REMS programs
- Support new referrals and maintain complete documentation
What We're Looking For
- Active Texas Pharmacist License (TSBP) in good standing
- 2+ years of experience with patient counseling and prescription verification
- MTM experience preferred
- Strong clinical judgment and communication skills
