Job Description

The Care Navigator assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. This role involves creating personalized care plans for long-term care members and educating them, along with their families and caregivers, on available services and benefits to meet their needs.

Responsibilities

• The team makes contractual contact with our members to assess for any unmet needs, services and addresses any barriers to health.
• Mainly telephonic assessments, calls to members or parents, providers.
